Payment Model:  choose one option.

  1. Use of Space for Recurring External Event, Class, or Workshop.
    Flat Fee arranged with our Event Coordinator.

  2. Use of space for Event, Class, or Workshop with Unity.
    50/50% split of revenue earned from event.
    Unity collects payments/registrants and issues payment to facilitator following the event.

  3. Use of Space for Nonprofit organizations for Event, Class, Workshop. 
    Special rate arranged.

  4. Use of Space for Single Event or Venue Rental
    Please complete our Venue Rental Request form

BY SUBMITTING THIS FORM AND RELATED IMAGES YOU ARE CONFIRMING THAT THE INFORMATION ON THIS FORM IS TRUE AND LEGAL TO USE AND THAT THE IMAGES YOU PROVIDE ARE EITHER LICENSED BY THE FACILITATOR OR PERMISSION FOR UNITY OF OAK PARK USE HAS BEEN GRANTED.
